The Brimfield Fire Department now participates in the VIAL OF LIFE program.

 

When we respond to any EMS incident, there is important routine information that we gather for our patient care reports.  This information includes the patient demographics (name, address, phone number), past medical history, current medications and doses and medical allergies.

 

Sometimes due to patient condition, this information is somewhat or completely unobtainable.  In the event of an emergency, sometimes the patient cannot think clearly enough to give this information.  Sometimes the patient is unconscious.  When paramedics do not have this vital information, it can make our job more difficult. 

 

When you take your part in the VIAL OF LIFE program, it makes all this information automatically available to us when we arrive at your home for an emergency.

 

Your Checklist:

  • Get your VIAL OF LIFE stickers and medical history forms from the Brimfield Fire Department (one for each person in your household)

  • Place one sticker on the upper right hand corner of the outside of your front door.

  • Place one sticker on the zip lock bag provided.

  • Fill out each form clearly.

  • Fold the forms and place them inside of the zip lock bag that has been provided. 

  • Place the bag on the left front side of your refrigerator with a magnet.

  • CONGRATULATIONS!  YOU ARE NOW A PART OF THIS SIMPLE LIFE SAVING PROGRAM!

 

If there are changes regarding medications, dosages or medical conditions, please update your form.  Please feel free to request a new form at any time.
